Ten years later, there are a total of roughly 500,000 certified Care Managers. In Japan, for a specialized position like this to be established and to proliferate in such a short time is almost unprecedented, and the conditions surrounding this position have yet to be made sufficiently clear.In 2011, however, the Long-term Care Insurance Law itself will be amended, and amendments to both medical treatments and long-term care compensation are scheduled for 2012. Under these circumstances, and with a view toward 2025, when the senior population in Japan is expected to peak, the structure of the Long-term Care Insurance System is scheduled to undergo a transition to a system of community-based comprehensive care. In the midst of these structural changes, the role to be played by Care Managers has not yet been clarified, and the very meaning of this position is being brought into question.The purpose of this paper is: (1) To present the background to the adoption of care management and care plans, which became the responsibility of Care Managers, as part of policies when the Long-term Care Insurance System was implemented in 2000; (2) to clarify the types of operations undertaken by Care Managers over the past ten years; and (3) to clarify users' response to the activities of Care Managers.
